Ingredients:  
1 cup sliced mushrooms
1/2 green pepper, diced
1 lb cream cheese
1/2 cup mayonnaise
1 tsp Worcestershire sauce
1 onion minced
2 hot banana peppers diced
2 cloves of minced garlic
1 Shrimp ring
1 tsp lemon juice
2 cups of shredded cheese

Directions:

Combine all ingredients (be sure shrimp tails are removed!)
Bake at 325 for 30 - 35 min.
Serve with crackers and or veggies
